H1N1 Flu (Swine Flu) Vaccine
Information for Students & Parents
The Health Department will be
holding
H1N1 vaccination clinics for K-12 students in each of the Allen
County school districts. A
county-wide schedule is available or call 419-228-INFO (4636)
for dates/times and locations. Students should plan to attend the
clinic in the school district in which they live.

page, you can complete the TWO STEPS to save time when you
bring your student for an H1N1 vaccination and
find
links to help you decide if the H1N1 vaccine is right for your
student.
PLEASE register your school district when you register your
student. Someone in the family must be designated as
the head of household, even if they will not be receiving
the H1N1 vaccine at this time. This also makes it easy to
register several members of the family

vaccinated

Print vaccine consent form- Print this form and complete it with a pen,
then bring it/send it with your student when he/she goes to receive the vaccine.
Please be sure the consent form is signed by a parent or
legal guardian. Consent forms cannot be accepted
if they are not signed.
