Services
Our office
provides certified copies of birth and death certificates,
legitimization of children, genealogy research, and birth
verifications. Effective July 1, 2003 Ohio Law prohibits the
issuance of uncertified copies. Records are available for
the State of Ohio only, December 1908 through the present.
Cost
Certified copies
of birth and death certificates are $27.00 each. No personal checks
are accepted. Fees must be paid when ordering. Please note: Effective
July 1, 2003 Ohio Law prohibits the issuance of uncertified
copies. Birth Registration Cards are no longer being issued
by order of the Ohio Department of Health Vital Statistics Division.
Fees must be paid
when the order is placed. Certified copies of birth and death
certificates are $27.00 each. We do not accept personal checks. You may pay by cash, money order,
or cashier’s check. We do not
take phone orders at this time.
How to get a
certified copy
To get a copy of
a birth or death certificate, we must have a written request. You
can do this by mail, in person, or on-line at
VitalChek
. At this time we do not offer requests by phone.
You may order on-line utilizing the
VitalChek website.
An additional usage fee will be charged.
By mail
You may print out
a form and mail it to the Allen County Health Department. The
information we need and our address are on the form you can print
below.

You can also
request a copy of a birth or death certificate by writing to us and
include the following information:
Birth
– full given name, date of birth, place of birth and parents names
(mother’s maiden name)
Death
– name of
deceased, date of death, city of death.
Fees must be
enclosed when the request is sent by mail. Certified
copies of birth and death certificates are $27.00 each. We do
not accept personal checks. You may pay by cash, money order
or cashier’s check. We process mail orders the same day they are
received. We cannot accept debit/credit card orders by mail.

In Person
You may come to
the Allen County Health Department to complete a written request for
a birth or death certificate. Please allow enough time for
processing documents, usually 10-15 minutes.
