Information
The
Board of Health has closed the Child Health Clinic as of May
2008. Each child seen in
this clinic has been contacted and encouraged
to seek a medical home with one of the providers listed in the
notification letter.
In order to
obtain a copy of a child’s medical records from this clinic,
please print and complete the “Request
for Release of Medical Records” and mail or bring this form
to the Allen County Health Department.
