Information
The Child Health Clinic offers a variety of services for children who
do not have a
family doctor. Our goal is to help all
children grow
up healthy. Children are
seen by appointment only.

Growth and Nutritional Assessment; Developmental Screening |

Vision and Hearing Screening |

Health Education |

Early discharge and 2 week check-ups for new babies |

Referrals to specialists when appropriate |

Social services available include: Psychological Assessment,
Screening, Referrals, and limited counseling |
Eligibility
Low income and Medicaid eligible children. We serve residents of
Allen County plus people who live on the Van Wert side of Delphos.
Cost
We use a sliding scale fee that is based on your family income.
Medical Card accepted.
No one denied service due to inability to pay.
How do I make an appointment?
Call the Health Department at 419-228-4457 and ask for the Child
Health Clinic. Children are seen by appointment only.
