Information
The Allen
County Family Planning STD Clinic offers comprehensive diagnosis and
treatment for sexually transmitted infections. Care is provided by
physicians and nurse practitioners supported by a professional and
caring staff. Education and counseling are also available.
Clinic Services
Walk-in clinic
hours: Each Tuesday from 9:00 - 11:00 a.m.
Appointments are available by calling 419-228-6154 for additional
exam times.
Education
regarding STD prevention and treatment, follow-up management, and
counseling is provided by trained staff.
If you would
like to be tested for HIV only, please call
419-228-4457 and ask to speak to Judy or Jodi.
Eligibility
Services are
available to both male and female clients. Children under 13 years
old must bring a parent or guardian for consent purposes.
Cost
Fees apply and
are assessed based on HOUSEHOLD INCOME information using the federal
poverty guidelines. Medicaid accepted.
Payment method -
Cash only. No
one denied service due to inability to pay.
