Information
The goal of this program is to offer comprehensive services for
education, and prevention, and to control the spread of sexually
transmitted diseases.
Clinic Services
Hours: 4:00 - 5:00 pm Tuesday -
Call before coming - Physician is not always available.
We provide physician or physician assistant exam and laboratory
studies: Currently able to screen for Chlamydia, gonorrhea, and
syphilis; screening for Hepatitis C when indicated. Symptoms must be present to identify and diagnose
genital herpes and genital warts.
The clinic provides treatment for the diseases mentioned above,
except for Hepatitis C, as ordered by the physician.
Education regarding STD prevention and treatment, follow-up
management, and counseling is also provided by trained staff.
HIV testing
is
not currently available through this clinic. If you would like to be
tested for HIV, please call 419-228-4457 and ask to speak to Judy or
Jodi.
Eligibility
The clinic is open to individuals 13 years of age and older. Out of
county residents accepted. No one is denied services due to
inability to pay.
Cost
Current copy of medical card or $25.00, however, no one denied
service due to inability to pay.
How do I make an appointment?
This is a walk in clinic.

clinic. Name, address, demographic information recorded in chart.
