Purpose:
Allen
County Family Planning provides quality confidential gynecological
and
contraceptive services to
men and women regardless
of financial
situations. Education and counseling
are provided regarding all
contraceptive choices and family planning issues.
Services
Family
Planning provides access to annual physical screening exams which
can include pap testing , breast health exams, screening tests for
Sexually Transmitted Infections and treatments for infections as
needed, HIV testing, pregnancy testing and counseling with referral
for care as needed, contraceptive products, and referral for other
healthcare services.
Care is
provided by an expert staff of licensed medical professionals
including Physicians, Nurse Practitioners and Nurse Midwives,
Registered Nurses and a very caring support staff who are available
to help answer all your healthcare questions.
Family
Planning also provides clinical experience for healthcare students.
Speaker services are available
Family
Planning services are available to anyone regardless of ability to
pay. All fees are based on a sliding fee scale.
Application
Procedures:
Initial
services begin by completing health and financial information forms
and a brief education/assessment visit with one of our staff nurses.
An appointment is then arranged for an exam by our provider staff.
Contraceptive supply visits are walk-in after the initial exam.
Other walk-in services include pregnancy testing, HIV testing,
weight and blood pressure checks and/or other counseling services.
Cost of Services
All
service fees are based on household income. If fees are applicable,
methods of payment are Medicaid or private insurance or cash only.
No one is denied care due to inability to pay.

grant monies, fees, donations, local funding programs.
