Information
The Family Planning Clinic at the Allen County Health Department offers pregnancy testing and
referral services. Proof of pregnancy is provided for
application for Ohio Medicaid coverage as neeeded.

from 8:00 - 11:00 am |

clients covered by the Medicaid program |
Eligibility
This program is available for pregnant women who are residents of
Allen County or surrounding counties
Cost
We use a sliding fee scale for service based on household income.
Medical card accepted. No one will be denied services due to the
inability to pay.
